Enrolling Patients in INSIGHT Remotely

Clients may have new patients enroll in INSIGHT remotely by emailing or texting them a link to their patient permission form.  This link varies by clinician and by program.  To download the correct enrollment link, please do the following:

1.       Log into INSIGHT and click on the Survey Link icon:

2.       If you are a Manager, please select the primary clinician for this patient.  If you’re a clinician, skip to #3.

3.       On the correct clinician’s survey link page, choose the program this patient is enrolling in:

4.       At the top of the page you might see “Adult Permissions” and or “Adolescent Permissions.” Under these headings, you will find the hyperlinks for the intake surveys. Select the hyperlink or “permission link” for the program that the patient is entering.Click on the 2 paper icon and select “Copy to clipboard”.

5.       Paste the link into an email or text message to the patient

6.       When the patient clicks on the link, their patient permission form will open up on their smartphone or computer for them to fill out.  Once they submit the patient permission, they will be taken directly to the appropriate intake questionnaire.
