How to Transfer Patients to Another Center

Instructions for Transferring Patients to Another Center Within Your Network of Care


Patients receiving treatment at your center should only be discharged when their entire treatment episode is complete. Therefore, Vista has created a unique software tool for transferring patients to other facilities within your organization. This can be useful if a patient is stepping down in care or transferring to a more appropriate facility.


Here are the instructions for completing a transfer:


  • First, navigate to the 'Patients' page. Then, click on the 3-dot menu on the right-hand side, and from the drop-down, select History.

  • On the 'History' page, there is a blue button on the top right called 'Transfer to another facility.' Click on that button, choose the facility, enter the date of transfer, and click save.

  • Once the originating center initiates the transfer, the next center must accept it. To accept, someone at the next center will need to navigate to the 'Survey Links' page. Scroll down until you see the 'Newly transferred patients' heading and click Confirm. After that, the transfer is complete.
