How to Transfer Patients to Another Meadows Facility

Instructions for Transferring Patients Within The Meadows Behavioral Healthcare's Continuum of Care

Patients receiving treatment in The Meadows network of care should only be discharged when their entire treatment episode is complete. If a patient is transferring to another facility (e.g., stepping down in care or transferring from Spero Center to Willow House), Vista has created a unique tool in the software for transferring from one Meadows location to another.

• Here are the instructions for completing a transfer, in this example, from The Meadows Texas (MEAD-TMTX) to The Meadows Outpatient Center, Dallas (MEAD-TX):

• First, navigate to the 'Patients' page. Once there, click on the 3-dot menu on the right, and from the drop-down menu, select History.

• On the 'History' page, there is a blue button on the top right called 'Transfer to another Meadows facility.' Click on that button, choose the facility, enter the date of transfer, and click save.
