Administering Patient Surveys
Overview
Types of Surveys
INSIGHT Administers three types of surveys:
- The Intake Questionnaire survey gathers baseline information on pre-treatment drug and alcohol use, quality of life, and addiction severity while screening for co-occurring disorders
- The Initial Progress Monitoring survey collects initial progress monitoring data
- Progress Monitoring surveys will collect information about patient symptoms on a weekly basis throughout their time with you in treatment. When a patient is determined to be stabilized, the Progress Monitoring surveys will be scheduled for every 2 weeks instead of every week.
Administering Intake Surveys
Intake surveys can be administered in the following ways ways:
- Using the "patient permission" link or copy patient link for remote sending
- From the EHR integration or copy the patient link for remote sending
Using the "patient permission" link
For centers without EHR integration, a staff member at the treatment center will guide the patient to the questionnaire, which includes a patient permission form and the intake questionnaire. - Click on the "Survey Links" icon located in the left-side navigation bar.

- Click on the name of the primary counselor assigned to the patient.

- Under "patient permissions," select the link for the appropriate program and/or level of care. You can also click on the "copy" icon to get a link to send via e-mail or text.

- You will then be logged out of the software, and the survey will open in a new tab.

Administering the intake from the EHR Integration
For treatment centers with EHR integration, patients' intake will be populated in the software.

Patient assigned to clinician and program
- If a patient is automatically matched with a clinician and program, their name will be hyperlinked in blue. Click on the hyperlinked name. You will be logged out of the software, and the patient permission screen will then open on another tab.
- You may also click the copy button next to their name to email the patient permission/intake questionnaire to the patient’s device.

Patient not assigned to clinician and/or program
- You will often need to set the counselor and/or program before enrolling a patient into Vista. Click on “Set Counselor” or “Set program,” highlighted in blue, to select which clinician/program the patient should be assigned to.


- To enroll the patient immediately, click “Enroll patient now.” You will then be logged out of the software, and the patient permission form will open in another tab.

***For all patients who have been assigned a counselor, you can also find the patient names listed under the "Patient Permissions Needed" heading under each counselor's survey links page

Administering progress monitoring (update) surveys
Update surveys can be completed in the following ways:
Push notifications to text or email
Vista can send automated texts or emails to your patients to remind them to complete their update survey. They can be sent:
- Every week on a certain day and time (e.g., Sundays at 1:30 p.m.)
- A certain number of days after the last completed survey (e.g., 14 days)
- Users can also push a reminder by:
- Clicking on the survey links menu, finding the patient in the clinician's page, and clicking "send request."


Patient Portal
- Treatment centers have a static link that patients can use to complete their survey. Treatment centers can bookmark it on shared devices (e.g., computers, tablets).
- Or centers can download a QR code to post in areas such as group rooms, the lobby, etc. Patients can quickly scan the QR code with their cell phone to access their next weekly survey.

- When a patient visits the portal, they’ll see a simple login screen (see screenshot below).

- The patient enters their name and date of birth to sign in.
- They’ll automatically be taken to their next available weekly update survey.
Software Link
Lastly, patient surveys can be administered from a link in the software. Users can visit the survey links page, find the assigned clinician, and then click on the patient's hyperlinked name.
