Patient Permissions for Adolescent Patients
INTRODUCTION
INSIGHT allows treatment centers flexibility around the issue of obtaining consent for minors. There are two setup options for minors in the software:
- Allowing minors to consent to participate in progress monitoring and post-treatment follow up
- Allow minors to provide assent and guardians to consent to have the minor participate in progress monitoring and post-treatment follow up

ADOLESCENT PATIENTS WHO NEED ADULT CONSENT
For patients that need adult consent, we have a two-part process.
Collecting permission from the adolescent patient (assent)
First, we collect the adolescent patient permissions (assent) to participate in progress monitoring and post-treatment outcomes. This process follows our regular enrollment process, which you can learn more about here: Administering Patient Surveys
Collecting permission from the guardian/parent
After an adolescent client completes their permission form, you can request that a parent or guardian complete their permission form by pressing a button. Parents or guardians must complete a permission form before the adolescent can proceed to the intake questionnaire. Note: To effectively send the parent permission form by text or email, the adolescent must enter a valid email and/or phone number on the adolescent permission form.
Once the adolescent has submitted their permission form, log back into the system and click on the "Survey Links" icon.
The patient's name will appear under the "Parent Permissions" list. To send the parent the permission, click on the "Send Request" button next patient's name.
Once the parent or guardian has completed the permission, the adolescent's name will appear under "Intake Questionnaire." on the "Survey Links" page. You may need to refresh the page to see the change. You can click on the patient's name to administer the Intake Questionnaire. If they have access to their phone or email, you can click "send request" to send the patient a link to the Intake Questionnaire.
Patients who can provide their own consent to participate in mental health treatment
Our system allows us to disable the parent permissions for your center for youth 12 and older. We will collect that information from your leadership during the initial set up and it can be changed at any time.
