Vista Notes
Overview
Vista Notes summarizes information collected from patients during intake and update surveys into data-rich paragraphs that can be used for:
- Writing intake summaries or biopsychosocial reports
- Developing treatment plan goals and objectives
- Documenting progress on treatment plan objectives
- Writing progress notes
- Articulating data and information for utilization review
- Providing evidence for other types of assessment processes, such as ASAM
How does the AI tool work?
To generate notes, Vista passes de-identified patient survey data from Vista’s software server to another HIPAA-compliant secure server, managed by AWS, containing the AI model, then returns the generated notes via API back to Vista’s software. None of the data is used to train the model. Vista provides instructions to the model to help craft how the tool works.
Clinicians must review and approve the notes before including them in the patient's health record.
Type of Vista Notes
Summary Report Notes (Intake Data):
These notes are designed for initial reports, including intake assessments, biopsychosocial reports, treatment plan development, or to supplement other assessment processes such as ASAM criteria. Utilizing the data collected in the intake assessment, Vista Notes provides a summary of the following categories:
- Presenting problem
- Substance Use
- Relapse Risk Factors
- Physical Health
- Emotional Health
- Social and Environmental Factors
Progress Notes (Update Survey Data):
Using the data from update surveys, progress notes are designed to be used when it’s necessary to report on patients' ongoing progress, such as progress on treatment goals or utilization reviews.
Vista Notes summarizes changes in patient scores using the unique measures for which they are being assessed. Progress notes summarize the data in the following formats:
- Summary
- Objective observations
- Progress patient has made
- Need for additional treatment
How to Use Vista Notes
Summary Report (Intake Data) Instructions:
- Navigate to the patient menu in the black menu bar on the left.
- Select a patient by clicking on the patient's name.
- In the patient dashboard, click “Summary Report” in the upper right.
- Click on the “Summarize” button for the section of your choice.
- You can use the text field below the summary to ask the tool to make any changes. Some examples of possible modifications are:
- Replace “patient” with “client” (or consumer, member, etc)
- Ask for more or less detail
- Make the summary longer or shorter
- Anything else you can think of!
Once you have written your request, select “refine.”
- To copy the note and paste it into your EHR or other documentation, click on the “copy” button in the upper right corner of the summary box.
Progress Note (During Treatment) Instructions:
- Navigate to the patient menu in the black menu bar on the left.
- Select a patient by clicking on the patient name.
- Click “Draft Note” on the patient dashboard on the upper right.
- You can use the text field below the summary to ask the tool to make any changes. Some examples of possible modifications are:
- Replace “patient” with “client” (or consumer, member, etc)
- Ask for more or less detail
- Make the summary longer or shorter
- Anything else you can think of!
Once you have written your request, select “refine.”
- To copy the note, click the copy button next to any other section or on the note navigation bar.
