Viewing Patient Reports

Overview of Patient Menu

Below is the patient's page. Here are further explanations for a few specific components:

Searching for Patients: You can use the search box (highlighted in red) to find patients by patient ID, patient name (either first or last name), counselor name, last survey date, or discharge date.

Sorting: Most of the fields are sortable. There is a sorting icon available to click, to the right of each heading.

Alerts: Alert levels are indicated by flags, with white flags for intake (historical) data and red flags for during treatment surveys. Visit the following link to learn more about one and two flag alerts: https://vista-research-group.helpscoutdocs.com/article/74-patient-alert-notifications-flags.

Date of Last Survey: The “updated” field indicates when the patient's record was last updated. Please note: while most of the time, this indicates the date of last report submission, there are times when their record has been updated with other information (e.g., updated patient permission, discharge date/readmission, etc.).

Discharge information: Under “Discharge date,” the patient’s status is listed; either active or the date they were reported as discharged. By clicking the three dots circled in red, you can update their status.

Counselor and Program Summaries: Counselor and program summaries allow you to see at a glance how all current patients for a given counselor or program are reporting feeling.

Current Patients/All Patients Buttons: By clicking on this button, you can show just the active patients or all patients on the roster.

Potential Discharges Button: Vista now offers a tool to help centers quickly review and update potentially overdue discharges.

Viewing Patient Reports

  • After searching for the specific patient, click the patient’s hyperlinked name or patient ID.

  • From the patient dashboard, you can view the patient's intake data and weekly update survey results to identify trends and assess whether the patient is progressing as expected.

  • From the top right toolbar, you can:
    • Select "Draft Note" to use Vista's AI-powered clinical documentation tool to summarize changes in patient scores using the unique measures for which they are being assessed.
    • Use "Change View" if a chart (e.g., depression or anxiety) isn't showing because the patient scored in the mild or no-symptoms range, or if you want to remove a chart from the view.
    • Download the report by clicking “Generate PDF.”
    • Review "Data Summissions" to see when surveys were submitted, and how the patient answered specific questions.
